Filing a Complaint with the Monitor

To initiate the complaint process, please complete the information fields below. Upon receiving your request to file a complaint, the Monitor team will respond within 72 hours to schedule a time for the Monitor and his team to review your concerns, the specific nature of the complaint, the scope of the complaint, and any specific timing issues that should be taken into consideration by the Monitor.

If the Monitor deems the nature of the complaint to be within the scope of the monitorship, the Monitor team will provide you with a document/data request that will stipulate the information necessary for you to provide to the Monitor. Upon receipt and review of the complaint and documents/data initially submitted pursuant to the Monitor’s request, the Monitor will proceed with his investigation which may include individual conference calls with each party, and if appropriate joint conference calls with the appropriate individuals at Cedars-Sinai/Huntington Hospital. As the investigation proceeds, the Monitor may make additional requests for information from the parties specific to the dispute. These supplemental requests may include data sets, emails, letters and/or other documents or data.

All productions of documents/data pursuant to the Monitor’s requests will be transmitted via upload to a secure portal provided by the Monitor team.

Information, including data and documents, submitted by a payer to the Monitor as part of the complaint process may be required to be bound by the protective order in place. As such, the payer is responsible for designating documents and other information submitted in connection with the complaint as confidential.
